Welcome to the Pointe Primary Care patient resources center! Our resource center is specially designed for you, our patients. Here, you will find a wealth of information to educate and advise you to ensure your next visit with us is pleasant. We believe in making healthcare accessible and efficient for you, our patient. By providing you with this information, we hope you will better understand your health, and your office visits will be more productive. 

New Patients

If you are a new patient to our practice, please download and complete our “Patient Packet”. Once completed and returned, we encourage you to schedule your first visit with us. We value your time and want to make your first visit as easy and efficient as possible. To expedite the process, we ask you to arrive 15 minutes early to finalize all paperwork. Please have with you:

  • Past medical records (including immunizations) and current medications
  • Health insurance card and driver’s license
  • Completed health forms

Pointe Primary Care is open on the following days:

  • M-F 8:00 AM-4:30 PM

Financial Policy

Pointe Primary Care accepts most major credit cards including Visa, MasterCard, Discover, checks, and cash. For self-pay patients, we offer a 20% discount when the balance is paid in full at the time of service.

Insurance

We accept Blue Cross, Aetna, Cigna, Humana, TriCare, VA (with prior authorization) Medicare, Medicaid (Amerihealth as secondary only) and most other non-managed care insurances. Please call to verify if we accept your insurance.

Policies

  • Deductibles, copays and co-insurances are expected to be paid the day of the visit.

  • No Shows for appointments are charged $50 first time, $100 second time, $200 third time with possible discharge. Due to state rules and policies, Medicaid patient cannot be charged a no show fee, however, patients will be discharged at the discretion of the individual provider for non-compliance.

  • All Telehealth appointments co-pays must be paid for 1 hour prior to the appointment.

To provide a safe and healthy environment for staff, visitors, patients and their families, Pointe Primary Care expects visitors, patients, and accompanying family members to refrain from unacceptable behaviors that are disruptive or pose a threat to the rights or safety of other patients and staff.
 
As a patient visiting our practice, please consider the following:
  • If you have any questions about the care or are unhappy with the service received in our office, please contact our practice administrator before you leave our office so that any clarifications about your care or the services you received can be addressed.
  • Please communicate all issues that you wish to discuss with the doctor at the time your appointment is scheduled, so that an appropriate amount of time can be allotted. If you do not do this in advance, another visit may be necessary so that the doctor can give all patients the time and quality of care they deserve.
  • Questions about your billing can be addressed by contacting our billing department at 302.684.2000 Monday, Tuesday, Thursday, and Friday.
  • Our practice follows a zero-tolerance policy for aggressive behavior directed by patients against our staff and providers.
  • Please be courteous with the use of your cell phone and other electronic devices. When interacting with any of our staff, please put your devices away. Set the ringer to vibrate before storing it away.
  • Adults are expected to supervise their children. 
 
The following behaviors are prohibited:  
 
  • Possessing firearms or any weapon
  • Intimidating, harassing, sexual harassment or innuendos towards staff or other patients
  • Making threats of violence through phone calls, letters, voicemail, email, or other forms of written, verbal or electronic communication
  • Physically assaulting or threatening to inflict bodily harm
  • Making verbal threats to harm another individual or destroy property
  • Damaging business equipment or property
  • Making menacing or derogatory gestures
  • Making racial or cultural slurs or other derogatory remarks
 
If you are subjected to any of these behaviors or witness inappropriate behavior, please report to any staff member. Violators are subject to removal from the facility and/or discharge from the practice.
